Fascia Pain: Causes, Symptoms and Treatment Options

Fascia pain describes discomfort that may involve the connective tissue surrounding and supporting muscles and other structures. However, pain in an area that feels tight or tender does not necessarily come from fascia alone. Muscles, tendons, joints and nerves may produce similar symptoms. Management therefore depends on identifying likely contributing factors through clinical assessment, followed by appropriate activity modification, exercise and other options where indicated.

What is fascia pain?

Fascia pain is a general term people may use for aching, tenderness or tightness thought to involve fascia, the connective tissue that surrounds and supports muscles and other structures. However, there is no single symptom that confirms fascia as the source of pain. A clinical assessment may therefore consider muscles, tendons, joints, nerves and other possible causes.

Fascia forms a network of connective tissue throughout the body. It surrounds muscles and helps different tissues move in relation to one another.

The term myofascial pain is also commonly used when pain involves muscles and surrounding connective tissues. Some people may have localised areas of muscle tenderness, sometimes referred to as trigger points.

However, muscle and soft-tissue pain can have many causes. Therefore, assessment should look beyond one tender area and consider the wider musculoskeletal system.

What does fascia pain feel like?

People commonly describe suspected fascia or myofascial pain as a deep ache, tightness, tenderness or sensitivity within a muscle or surrounding soft tissue. The discomfort may remain localised or sometimes feel as though it spreads to another area. Symptoms often vary with movement, activity, posture and pressure on the affected tissues.

Possible symptoms include:

  • A dull or aching sensation
  • Local tenderness
  • Muscle tightness
  • Sensitivity when pressure is applied
  • Reduced comfortable movement
  • Pain after repetitive activity
  • Discomfort after prolonged sitting or standing
  • Symptoms during certain exercises or sporting movements

However, these symptoms are not specific to fascia. Tendon conditions, joint problems and nerve irritation can sometimes produce similar sensations.

Where can fascia pain occur?

Fascia and connective tissue exist throughout the body, so myofascial-type discomfort can occur in many areas. Common sites include the neck, shoulders, upper and lower back, hips, thighs and calves. However, the location of pain alone cannot identify which tissue is responsible.

Common areas include:

Neck and shoulders
Symptoms may occur after prolonged desk work, repeated upper-body activity or changes in training.

Upper and lower back
Back discomfort may involve muscles and surrounding soft tissues, although joints, discs and nerves can also contribute.

Hips and buttocks
Pain in this region may arise from several structures, including muscles, tendons, joints and the lumbar spine.

Thighs and calves
Running, jumping or changes in exercise load may contribute to muscle soreness and soft-tissue symptoms.

Because several structures often sit close together, clinical evaluation may help determine the likely source of persistent pain.

Why does my fascia hurt?

Pain that feels as though it comes from fascia may develop alongside muscle overload, repetitive activity, changes in exercise, prolonged positions or an injury. However, persistent musculoskeletal pain is often influenced by several factors rather than one tissue alone, so the cause should not automatically be attributed to “tight fascia.”

Possible contributing factors include:

  • A sudden increase in exercise
  • Repetitive occupational movements
  • New sporting activities
  • Muscle strain
  • Reduced strength or conditioning
  • Prolonged static positions
  • Changes in movement after an injury
  • Inadequate recovery between training sessions
  • Persistent musculoskeletal conditions

Sleep, stress and general health can also influence how a person experiences persistent pain.

Therefore, assessment may consider both the painful area and factors elsewhere in the body that could affect movement or loading.

Trigger Points

Is fascia pain the same as muscle pain?

Not exactly. Fascia and muscle are different tissues, although they work closely together and can be difficult to separate when assessing symptoms. For this reason, healthcare professionals may use terms such as myofascial pain when symptoms appear to involve muscles and surrounding connective tissues rather than describing fascia as the only source.

Muscles generate force and movement. Fascia provides connective support around and between muscles and other anatomical structures.

Meanwhile, tendons connect muscles to bones, ligaments help stabilise joints, and nerves transmit signals throughout the body.

Because these structures interact, pain during movement may have more than one contributing factor.

Can “tight fascia” cause pain?

People often use the phrase “tight fascia” to describe stiffness or restricted movement, but the sensation of tightness does not necessarily mean that fascia has physically shortened or become damaged. Muscle tension, joint stiffness, sensitivity, activity load and other factors may contribute to the same feeling.

For example, a runner with calf tightness may have:

  • Muscle fatigue
  • A recent increase in running distance
  • Reduced ankle movement
  • Tendon-related symptoms
  • A muscle injury
  • Nerve-related symptoms

Similarly, shoulder tightness may relate to the neck, shoulder joint, rotator cuff or surrounding muscles.

Therefore, treatment should not focus on “releasing fascia” without first considering the wider clinical picture.

Who may experience myofascial or soft-tissue pain?

Myofascial and soft-tissue symptoms can affect both active and less active people. They may occur after repetitive work, prolonged postures, unfamiliar exercise or increased training. Athletes may also develop symptoms when training volume, intensity or technique changes faster than their body can comfortably adapt.

Activities that can place repeated demands on muscles and soft tissues include:

  • Running
  • Cycling
  • Swimming
  • Tennis
  • Golf
  • Badminton
  • Weightlifting
  • Gym training
  • Football and field sports

Occupation may also influence symptoms. For example, prolonged computer use can contribute to neck and shoulder discomfort, while repetitive manual work may affect the forearm or upper limb.

Importantly, these activities do not automatically cause fascia problems. They simply provide useful context during assessment.

How is fascia pain assessed?

Assessment usually starts with a discussion about where the pain occurs, how it started and which movements or activities affect it. A musculoskeletal examination may then assess movement, strength, tenderness and function while also checking for other possible causes involving joints, tendons or nerves.

Assessment may include:

  • Medical and injury history
  • Sporting and exercise history
  • Location and pattern of symptoms
  • Range of movement
  • Muscle strength
  • Functional movements
  • Areas of tenderness
  • Neurological assessment where relevant
  • Review of work or training demands
  • Dynamic ultrasound assessment where relevant

A clinician may also assess neighbouring regions.

For example, persistent shoulder symptoms may require examination of both the shoulder and neck. Similarly, buttock or thigh pain may require assessment of the hip, lower back and nervous system.

Do I need an MRI or ultrasound for fascia pain?

Most people with muscle or myofascial-type pain do not automatically require imaging. Clinical assessment often provides the initial information needed. However, imaging may be considered when symptoms suggest another condition, follow significant injury, remain unexplained or require further evaluation before deciding on management.

Depending on the clinical question:

X-rays mainly assess bones and joints rather than fascia.

MRI can provide information about muscles, tendons, bones, joints and other soft tissues.

Musculoskeletal ultrasound may assess selected muscles, tendons and other superficial soft-tissue structures in real time.

Where fascial or myofascial symptoms are suspected, ultrasound can be particularly useful because it looks at the tissue while it is moving. In selected cases it may be used to check the thickness of the fascia, comparing the affected side with the opposite side where appropriate. Because the scan is performed in real time, it can also assess the movement and gliding of the tissue layers as a muscle contracts or a limb is moved, which may show whether neighbouring layers slide normally against one another or appear restricted.

Ultrasound can also help identify a soft-tissue or muscle tear and, when a procedure is being considered, allows a treatment to be guided accurately to the intended target.

Importantly, imaging findings need to match the clinical picture. A scan finding does not always explain why someone experiences pain.

What treatment options may help fascia pain?

Management depends on the likely source of symptoms and contributing factors. Options may include education, temporary activity modification, gradual exercise and rehabilitation. The aim is generally to support comfortable movement and function rather than assuming that painful tissue must simply be stretched or “released.”

Understanding which movements increase symptoms can help guide temporary adjustments without necessarily stopping all activity.

Exercise and rehabilitation

Depending on the assessment, rehabilitation may include:

  • Mobility exercises
  • Progressive strengthening
  • Movement retraining
  • Gradual loading
  • Sport-specific exercises
  • Return-to-activity planning

Exercise is commonly included within management approaches for myofascial pain.

Medication

Medication may sometimes be considered depending on the condition and the individual’s medical history. This should be discussed with a healthcare professional.

Other procedures

Depending on the individual’s condition and clinical assessment, certain procedures may be considered as part of a broader management plan.

Ultrasound-guided procedures

Where a procedure is judged appropriate, ultrasound guidance allows a needle to be directed accurately to a specific target while the surrounding structures are watched in real time. Two options that may be considered for selected soft-tissue or fascial problems are hydrodissection and platelet-rich plasma.

Ultrasound-guided hydrodissection involves injecting fluid under direct vision to gently separate tissue planes that appear restricted or adherent, or to free a nerve that lies closely against surrounding fascia. Where reduced gliding between layers has been identified on assessment, the aim is to help restore more normal movement between them.

Platelet-rich plasma (PRP) uses a small volume of the patient’s own blood, prepared to concentrate the platelets, which is then placed under ultrasound guidance at a selected site. It may be considered for certain muscle or soft-tissue tears where this approach is judged suitable following clinical and imaging assessment.

Prolotherapy involves injecting a mild irritant solution, most commonly dextrose, to prompt a local healing response. Under ultrasound guidance it may be considered for selected persistent soft-tissue or connective-tissue symptoms that have not settled with rehabilitation, where it is judged suitable for the individual.

Related image-guided options, such as targeted soft-tissue injections, ultrasonic tenotomy and shockwave therapy, may also be discussed where relevant. As with any procedure, suitability, expected benefits, limitations and alternatives are considered on an individual basis rather than offered routinely.

However, procedures should not replace an assessment of contributing factors or an appropriate rehabilitation programme.

Can trigger point injections help myofascial pain?

Trigger point injections are one option that may be considered for selected patients with myofascial pain. They involve placing a needle into an identified tender area, sometimes with medication. However, they are not appropriate for every person with muscle or fascia-related symptoms, and their role depends on the clinical assessment and broader management plan.

Trigger point injections are among the approaches used clinically for myofascial pain, alongside physical therapy and exercise.

Before considering a procedure, the clinician may discuss:

  • The likely source of symptoms
  • Other available management options
  • Expected limitations
  • Potential risks
  • Rehabilitation after the procedure

The decision should reflect the individual’s clinical findings rather than the presence of tenderness alone.

What exercises may help fascia pain?

There is no single “fascia exercise” suitable for everyone. Exercise selection depends on where symptoms occur, the movements that aggravate them, current strength and the person’s work or sporting requirements. A programme may combine mobility, strengthening and gradual exposure to previously uncomfortable activities.

For example, rehabilitation may involve:

  • Neck and shoulder strengthening for upper-body symptoms
  • Trunk exercises for back-related symptoms
  • Hip strengthening for lower-limb movement
  • Calf strengthening for running-related symptoms
  • Gradual resistance training
  • Mobility exercises where movement is restricted

Progress should reflect symptoms and function rather than following a fixed timeline.

How long does fascia pain take to improve?

There is no standard recovery time for fascia or myofascial pain. Some activity-related symptoms settle as training or daily activity is adjusted, while persistent symptoms may require a longer period of rehabilitation. Recovery can vary according to the cause, symptom duration, physical demands and other contributing factors.

Rather than focusing only on a specific number of days or weeks, progress can be assessed through changes such as:

  • Improved comfortable movement
  • Greater strength
  • Better tolerance of daily activities
  • Gradual return to exercise
  • Reduced disruption to work or sport

Persistent symptoms should be reassessed if they do not progress as expected.

When should I seek medical assessment for fascia pain?

Consider medical assessment when pain persists, repeatedly returns, limits movement or affects work, exercise, sleep or daily activities. Assessment is also appropriate when the cause is unclear or symptoms include weakness, numbness, significant swelling or follow a substantial injury.

More urgent assessment may be appropriate if pain occurs with:

  • Significant trauma
  • Sudden loss of strength
  • Progressive weakness
  • New or worsening numbness
  • Fever or feeling significantly unwell
  • A hot or markedly swollen joint
  • Unexplained severe pain
  • Loss of normal limb function

These symptoms may indicate a condition other than uncomplicated muscle or myofascial pain.

Frequently Asked Questions About Fascia Pain

Can fascia pain feel like a knot?

People sometimes describe tender muscle areas as knots. These sensations may occur within muscles and surrounding connective tissue, particularly after repetitive activity or prolonged positions. However, feeling a knot does not confirm a specific structural problem with fascia. Persistent or recurring tender areas may benefit from assessment to consider muscle loading, movement, nearby joints and other possible sources of symptoms.

Is myofascial pain the same as fibromyalgia?

No. Myofascial pain and fibromyalgia are different clinical concepts. Myofascial pain commonly refers to more localised muscle and soft-tissue symptoms, while fibromyalgia involves widespread pain and other symptoms. Because persistent pain can have several causes, people with widespread or unexplained symptoms should seek appropriate medical assessment rather than assuming the problem comes from fascia.

Can massage help fascia pain?

Massage may temporarily change how tight or sore an area feels for some people. However, the response varies, and massage does not necessarily address factors such as strength, training load or movement tolerance. When symptoms persist, it may be more useful to consider massage as one possible component of a broader management approach rather than as a standalone solution.

Is stretching good for fascia pain?

Gentle stretching may feel comfortable for some muscle and soft-tissue symptoms, particularly when movement feels restricted. However, repeatedly forcing a painful area into a strong stretch may aggravate symptoms. The appropriate type and amount of stretching depends on the underlying condition, so persistent pain may require clinical assessment and an individualised exercise plan.

Can fascia pain happen after exercise?

Yes. Muscle and surrounding soft tissues may feel sore after unfamiliar or demanding exercise. However, exercise-related pain can also result from muscle strain, tendon problems, joint conditions or other injuries. Symptoms that are severe, associated with swelling or weakness, or that continue beyond the expected period of post-exercise soreness may require assessment.

Does fascia pain show on an MRI?

MRI can show many soft-tissue structures, but there is no single MRI finding that confirms general “fascia pain.” Imaging is usually selected to answer a specific clinical question. Therefore, an MRI may be considered when a clinician suspects another muscle, tendon, joint, bone or soft-tissue condition rather than simply because an area feels tight or tender.

Can fascia pain cause nerve symptoms?

Musculoskeletal pain and nerve-related symptoms can sometimes occur in similar areas, but numbness, tingling, burning sensations or weakness may suggest nerve involvement. These symptoms should not automatically be attributed to fascia. Clinical assessment can examine sensation, strength, reflexes and relevant nerve pathways where necessary.

Can fascia pain come back?

Muscle and soft-tissue symptoms may recur, particularly when activity demands change or contributing factors remain. A recurrence does not necessarily mean that tissue has been damaged again. Reviewing exercise load, strength, movement, recovery and the original diagnosis may help determine why symptoms have returned and whether further assessment is appropriate.

Can injections such as hydrodissection, PRP or prolotherapy help fascia problems?

These injection options may be considered for selected people, usually after clinical assessment and, where appropriate, ultrasound. Ultrasound-guided hydrodissection aims to free tissue planes or a nerve where gliding appears restricted; platelet-rich plasma may be considered for certain muscle or soft-tissue tears; and prolotherapy is sometimes used for persistent soft-tissue or connective-tissue symptoms. Trigger point injections are a further option for some people with myofascial pain. None of these is suitable for everyone, and they are generally used alongside rehabilitation rather than in place of it. The most appropriate choice depends on the individual’s diagnosis, symptoms and clinical findings.
